4A: no large S printed on the form and no entry for Time in the lower left corner;
4B:
no large S printed on the form but two lines for Time in the lower left corner;
4C: large S printed on the form.
Distinctive characteristics of this form:
- centre line "Received..." ends with "message";
- line above sender details ends with "Tasmania".
